The City of Long Beach Vital Records Program maintains birth and death records for events occurring in Long Beach. Birth and Death Certificate requests can be made by mail or in-person for events that occurred within the past five years at the Long Beach Department of Health and Human Services.

Monday - Friday from 8 a.m. to 4 p.m. (Closed from noon to 1 p.m.)
We do not take appointments.
